AP: Benghazi attack review finds systematic State Dept. failures

An independent panel investigating the Sept. 11 attack in Libya that killed a U.S. ambassador and three others has determined that systematic management failures at the State Department led to inadequate security leaving the diplomatic mission vulnerable. The review board also determined that no individual officers breached their duties.
